* If your child will require any medications or health services in the upcoming school year (2020-2021) contact the school office to advise.

ALLERGY INFORMATION:

If your child is diagnosed with a severe allergy that requires an epinephrine, please contact the School Nurse so that a cooperative plan may be developed to maintain your child's safety in school.

Reinforce with all children that sharing and trading food in school is NOT ALLOWED for the safety of everyone.

**All foods provided for classroom consumption must be STORE BOUGHT with a complete LIST OF INGREDIENTS.

ASTHMA:

If your child will require asthma medication (including rescue inhaler) during school hours, an ASTHMA ACTION PLAN must be completed by your child's healthcare provider and signed by the parent/guardian.  

Asthma Action Plan forms are available through the school health office and also may be downloaded from the PACNJ website:

Administration of Medications in School :

If your child requires the administration of ANY medication, either prescription or over the counter medication, including, but not limited to Motrin, Tylenol, Allergy Medications, cough drops, etc., the proper documentation is required.  

Please call the School Health Office (732)- 237-9601, as soon as possible.
