Required Immunizations

Students entering Grade 6 in September 2020 must show proof of the following immunizations before  September 2020

-Tdap 

-Meningitis Immunization

• Please provide documentation as soon as possible.
• Documentation must be received before September 30, 2020.
• Students without proper documentation will be excluded from school October 1, 2020 until documentation is provided.

Kindergarten Students entering September 2020

• Please provide the health office with a completed Physical Examination
• Complete Immunization Records
• Please provide all necessary information to the health office as soon as possible
• Documentation must be received before September 30, 2020.
• Students without proper documentation will be excluded from school October 1, 2020 until documentation is provided.

Transfer Students:

Please provide a physical examination report.

Physical Examination must be dated within a year of pupil's start date.
